30-Day Readmission Risk Model

Predictive model scoring inpatients for 30-day readmission risk using demographics, diagnosis history, prior utilization, length of stay, and discharge disposition extracted from the clinical data warehouse. Built in Python with scikit-learn, validated against two years of historical encounters, and surfaced to case managers through a daily-refreshed dashboard so high-risk patients receive discharge-planning interventions before they leave.
